Patients often experience delays because they do not know where to go, how to access services, or how to complete referrals and follow-ups. These barriers lead to missed appointments, late treatment, worsening health conditions, and unnecessary suffering-especially for vulnerable and low-income individuals.
By guiding patients through the healthcare system, this project removes confusion, reduces missed care, and ensures patients receive timely and appropriate treatment.
This program will improve continuity of care, reduce preventable complications, and strengthen trust in healthcare services. Over time, it will lead to earlier treatment, better health outcomes, and more efficient use of healthcare resources across Ghana.
