Summary

This project will support Health Horizons International's efforts to build and strengthen clinical capacity on COVID-19 mitigation and community health outcomes. This will affect 20,000 people living with chronic diseases in socioeconomically and politically disadvantaged communities in the Caribbean through the training, development, and empowerment of community health workers that will provide accessible, affordable, and quality health care services.

Challenge

Our communities face multiple barriers to health care, including a shortage of clean water and sanitation, gender inequality, lack of clinical infrastructure, inaccessible sustainable housing, little education, and institutionalized discrimination. The Covid-19 spread has widened the gap for vulnerable groups living with chronic diseases to have access to affordable and accessible health care.

Solution

HHI trains and empowers community health workers living in rural villages to deliver primary care services like glycemia and blood pressure testing, vaccination, covid-19 testing and management, specialist referral, home visits to inaccessible locations, mobile clinics, medication delivery, counseling and mental health support, access to clean water, and community health education activities for more than 20,000 people.

Long-Term Impact

This project will provide the necessary education to 30 local Community Health Workers to deliver health care services for the 20,000 underserved people in rural Dominican Republic affected by Covid-19. By improving community health and strengthening the health system and services, we will decrease the incidence of NCDs and chronic infectious diseases, as well as tropical diseases and Covid-19 cases.
