This project provides medical, financial, and educational support to help patients control their blood pressure and maintain heart health.
Without proper care, hypertension and heart disease can lead to strokes, heart attacks, kidney failure, and premature death. Low-income patients often skip medications, avoid doctor visits, or lack access to healthy food and monitoring tools, worsening their condition and increasing preventable complications.
By providing medications, monitoring tools, and lifestyle education, this project ensures patients can control hypertension and manage heart disease. Consistent support prevents complications, reduces hospitalizations, and improves overall health and longevity.
