Promote Healthy Communities in Kenya: CHW Program

We have exciting news about HopeCore’s Community Health Promoters (CHPs) Program! I am talking about 1265 CHPs across Tharaka Nithi County, all trained on select health topics thanks to HopeCores' bi-monthly sessions. They even have flipbooks packed with key information to share with their communities. CHPs conducted 21,503 home visits, allowing them to connect with families where they live, understand their specific health needs, and provide them with personalized health education and resources. It's about reaching people where they are, addressing their health concerns, and empowering each community member to take charge of their health.

Now let's talk about tackling high blood pressure! CHPs screened 28,257 people during home visits during this quarter. These blood-pressure screenings help to identify at-risk individuals and connect them with healthcare facilities for proper treatment. Out of those who were screened, 4,290 individuals were diagnosed with high blood pressure. CHPs referred 2,370 of them to health facilities to get the needed treatment and the rest were educated on measures to stabilize their blood pressure for example, if it is low they are asked to take a lot of water and fluids. Early detection of high blood pressure is critical to preventing serious health complications. Imagine the impact – catching these health issues early and getting the proper care can make all the difference!

Speaking of training, HopeCore held a flipbook campaign and refresher training on hypertension for 439 CHPs. The CHPs from Tharaka were so fired up after their blood pressure training that they requested diabetes training so they could serve their communities with even more knowledge and impact.

To ensure support supervision for our CHPs, HopeCore’s Community Health Officers provided on-the-ground support. This supervision ensures that CHPs provide accurate information, follow best practices, and address challenges. It's like having a coach in the corner, helping them be the most effective health promoters they can be!

During home visits, our CHPs saw 849 pregnant women, providing prenatal education, monitoring the mother's health, and identifying potential problems early on. They connected mothers with resources like antenatal care services and provided emotional support throughout the pregnancy. These visits ensure that moms have the knowledge and support they need for healthy pregnancies and deliveries.
